Job Description
A beautiful rehabilitation facility in Petersburg VA has an exciting opportunity for a Director of Case Management to join their team!
- Pay: $95000 $105000 / annually
Director of Case Management Opportunity:
- Fulltime directhire position
- Schedule: Mon Fri day shift
- Open to both registered nurses and social workers (must be licensed)
Director of Case Management Responsibilities:
- The Director of Case Management will oversee daytoday operations and human resource management of the Case Management department.
- Supervise the interdisciplinary plan of care and discharge planning process focusing on census management patient care outcomes and key care indicators
- Provide guidance and support to Case Managers and staff including training on caseload management regulations policies and operational procedures
- Build relationships with insurance companies employers case management firms and healthcare networks
Director of Case Management Qualifications:
- BSN or BSW
- Must be either a Licensed Registered Nurse or a Licensed Social Worker
- 2 years of leadership experience in a hospital setting
- 3 of hospitalbased Case Management experience including Utilization Review and Discharge Planning experience
- Current CCM or ACMTM certification is preferred
