Job Description

As a Case Manager within a dedicated team you will be responsible for providing comprehensive support to patients. You will ensure effective coordination of healthcare and wellbeing services while aligning with national standards and regulatory requirements. This role is crucial in connecting patients with the appropriate resources and facilitating access to quality healthcare.

Key responsibilities

  • Oversee case management processes monitor individual cases and provide direction to resolve complex issues while maintaining accurate documentation. 
  • Monitor the progress of individual cases providing direction triage and assistance as needed to resolve complex issues. 
  • Identify and mitigate health risks ensuring alignment with public health standards through regular reviews of health risk management strategies. 
  • Coordinate mental health promotion activities and monitor their effectiveness adjusting programs as necessary to meet patient needs 
  • Work closely with healthcare providers and stakeholders to facilitate communication between patients and their healthcare options providing expert advice for complex cases. 

Qualifications :

You are dedicated to making a meaningful impact in the lives of individuals with complex needs. You bring extensive experience in case management coupled with strong leadership abilities to guide and develop a team of professionals. Your excellent communication and interpersonal skills enable you to build rapport with clients families and multidisciplinary partners effectively. 

 

  • Bachelors degree in Social Work Nursing Healthcare Management or a related field. 
  • Minimum of 5 years of experience in case management healthcare coordination or a related field.  
  • Ability to work collaboratively with a diverse range of stakeholders. 
  • Strong analytical skills to identify and address health risks and issues. 
  • High level of accuracy in recordkeeping and reporting. 
  • Ability to adapt to changing circumstances and respond effectively to emerging health risks or issues. 
  • Knowledge of Australias public health system and relevant healthcare regulations
