Job Description

In this role you will lead a dedicated team of case managers providing guidance training and supervision to ensure effective service delivery. You will collaborate with multidisciplinary teams and community partners to develop and implement personalised care plans that address the unique challenges faced by clients.  

Key responsibilities

  • Manage coordinate and ensure accurate uptodate recordkeeping of patients healthcare needs and services provided. 
  • Supervise and lead a team of case managers providing guidance support and mentorship.  
  • Foster a collaborative and supportive team environment encouraging professional growth and development among team members. 
  • Monitor the progress of individual cases providing direction and assistance as needed to resolve complex issues. 
  • Regularly review and update health risk management strategies to maintain alignment with public health standards. 

Qualifications :

You are dedicated to making a meaningful impact in the lives of individuals with complex needs. You bring extensive experience in case management coupled with strong leadership abilities to guide and develop a team of professionals. Your excellent communication and interpersonal skills enable you to build rapport with clients families and multidisciplinary partners effectively. 

  • Bachelors degree in Social Work Nursing Healthcare Management or a related field. 
  • Minimum of 5 years of experience in case management healthcare coordination or a related field.  
  • Ability to work collaboratively with a diverse range of stakeholders. 
  • Strong analytical skills to identify and address health risks and issues. 
  • High level of accuracy in recordkeeping and reporting. 
  • Ability to adapt to changing circumstances and respond effectively to emerging health risks or issues. 
  • Knowledge of Australias public health system and relevant healthcare regulations
