Job Description

In this role as a Complex Case Manager Resident you will be part of a dedicated team of case managers providing guidance training and supervision to ensure effective service delivery. You will collaborate with multidisciplinary teams and community partners to develop and implement personalised care plans that address the unique challenges faced by clients. This role requires strong organizational skills empathy and a deep understanding of the healthcare challenges faced by individual. 

Key responsibilities

  • Manages patient files ensures compliance with policies handles legal documentation and communicates with external agencies. 
  • Ensures that policies and procedures are being followed including safety protocols guidelines and facility regulations. 
  • Coordinates communication between different departments and external agencies. 
  • May supervise administrative staff and ensure operational efficiency in terms of case handling scheduling and facility management. 
  • Responsible for data collection and reporting tracking patient statuses and creating reports for internal or external review.

You bring extensive experience in case management coupled with strong leadership abilities to guide and develop a team of professionals. Your excellent communication and interpersonal skills enable you to build rapport with clients families and multidisciplinary partners effectively. 

  • Bachelors degree in Social Work Nursing Healthcare Management or a related field. 
  • Experience in case management is essential with a minimum 5 years in similar environments such as social services mental health or correctional settings. 
  • Ability to work collaboratively with a diverse range of stakeholders. 
  • Strong organisational and administrative skills to manage complex cases. 
  • High level of accuracy in recordkeeping and reporting. 
  • Ability to adapt to changing circumstances and respond effectively to emerging health risks or issues. 
  • Knowledge of Australias public health system and relevant healthcare regulations
