Job Description
Assist healthcare teams in managing patients medication therapy collaborating with pharmacists nurses and physicians.
Responsibilities:
Document and resolve medication discrepancies.
Educate patients on medication use and adherence.
Monitor adherence and coordinate refills authorizations and transitions.
Maintain accurate patient records and support care plans.
Ensure regulatory compliance and contribute to quality improvement.
Qualifications:
Certified Pharmacy Technician (CPhT) or licensed.
Experience in pharmacy or care management.
Knowledge of pharmacology and EHR systems.
Strong communication skills and attention to detail.
Background check required.
