The Medication Therapy Management Specialist focuses on optimizing medication therapy for patients through comprehensive reviews education and collaboration with healthcare teams.
Key Responsibilities:
- Conduct thorough medication reviews and essments to identify potential drugrelated problems.
- Develop and implement individualized medication therapy plans in collaboration with patients and healthcare providers.
- Provide patient education on medication usage adherence and lifestyle modifications.
- Monitor patient progress and adjust therapy based on clinical outcomes.
- Doent and report medicationrelated interventions and outcomes.
Requirements
- Doctor of Pharmacy (Pharm.D.) degree with training in medication therapy management.
- Board certification in medication therapy management (BCMTMS or equivalent) preferred.
- Strong ytical and clinical ss.
- Effective communication and interpersonal ss for patient and provider interactions
