Job Description
Grand Rapids Hospital is looking for a Locum Ophthalmologist!
Details:
Practice Setting: Hospital based
Reason for Coverage: Trauma
Dates of Coverage: January July
Call Schedule: one week a month
Required Clinical Areas: NonSurgical Surgical/Invasive
Average Number of Patients Per Day Hospital: 12 per month
Will Other Ophthalmologists Provide Coverage During Locums Coverage Dates Yes
What Percentage of the Practice is Neonate Pediatric Adolescent Adult Geriatric: 100% Adult/Geriatric
What Percentage of the Practice is NonSurgical Surgical Laser: 50%
Documentation System/EMR: Epic
Trauma Level/Designation: Level 2
Estimated Credentialing Timeframe: 3 months
Facility will reimburse Flight Rental Car Mileage Billed at the IRS Standard Rate
**Trinity Health does not reimburse for credentialing fees**
Requirement description :
- Board Certified Ophthalmologist Required
- MI license Required
- BLS/ACLS Not required unless performing moderate or deep sedation
- Conversion rate cap of 25k Required
CERTIFICATION REQUIREMENTS :
- Board Certified
STATE LICENSE REQUIREMENTS :
- Michigan
ADDITIONAL LICENSE REQUIREMENTS :
