Work Schedule
Monday Friday 8 to 12 hour shifts between 6:00am and 6:30pm
OnCall required on rotational basis with other staff plus minimal callback
Job Specific Details and Tasks: The duties for the HCW include but are not limited to the following:
- The contractor shall provide all management tools supplies equipment and labor necessary to operate and maintain qualifications for a Registered Vascular Technologist.
- The services provided in this contract will be utilized to supplement active duty Air Force providers. Performance shall be according to the requirements contained in this performance work statement (PWS) and professional standards of The Joint Commission (TJC) National Patient Safety
- Connections/interfacing of cardiac ultrasound cardiac device programmers into cath lab suite display systems
- Provide assistance in other cardiopulmonary service areas at the discretion of the Cardiology Element Chief.
- Maintain cardiac catheterization lab
- Assist with ordering and maintaining supplies for the cardio
- Excellent oral communication skills; communicates with coworkers in a respectful manner. Refrains from displaying anger resentment cynicism or blaming others for adverse Will not threaten violence retribution or litigation.
- Establishes and/or utilizes a checklist to monitor the physical plan of the Cardiac Catheterization Laboratory and equipment for conformity to all national standards for ensuring safety for the patient and all personnel.
- Cardiac Catheterization Laboratory Technician may be assigned to serve as a shift lead tech who is responsible for ensuring housekeeping of cath lab is done for the regular duty shift. Housekeeping responsibilities would include daily tasks such as crash cart checks room temperature checks ensuring any equipment or supplies are adequate. Cardiac Cath Lab Shift lead tech of the day would work alongside the Cardiac Cath Lab charge nurse of the day (or the shift) and must report any housekeeping questions issues concerns to the Cardiac Cath Lab Officer In Charge or the FRED.
- Employee requires extensive knowledge and handson experience in evaluative diagnostic and interventional modalities during both invasive and noninvasive cardiovascular procedures. Performs a wide variety of highly complex potentially lifethreatening procedures associated with cardiac catheterization which may or may not have standard instructions for procedures to produce diagnostic images in any modality available in the catheterization lab. Indepth knowledge of current and legacy catheterization digital imaging modalities in order to perform complex examinations and procedures in truly emergent care scenarios. These procedures include right and left heart catheterization percutaneous transluminal coronary angioplasty coronary and vascular stenting rotational coronary atherectomy Eximer Laser Coronary Angioplasty (ELCA) coronary and peripheral lithotripsy manual and mechanical thrombectomy IntraAortic Balloon Pump (IABP) insertion and management cardiac device (pacemaker defibrillator Cardiac Resynchronization Therapy (CRT) loop recorder) implantation and interrogation biplane selective coronary angiography left ventricular aortic root and arch angiography pulmonary angiography
- selective arteriograms and special intervention angiography with the use of pharmaceutical agents and various forms of physiologic stress. Procedures also include intracoronary anticoagulant administration and percutaneous transluminal coronary angioplasty in both stable and unstable acute myocardial infarction patients. Based on extensive experience in performing the above procedures and on knowledge of major systems as well as cardiac anatomy and physiology employee selects appropriate patient position for optimum display of angiographic anatomy guides choice of appropriate equipment and directly controls the volume of contrast media administered. Must possess indepth knowledge of cardiac arrhythmias and knowledge of treatment methods to correct irregularities. Employee independently determines radiographic exposure rate and frame rate as appropriate for each case based on procedural requirements and patient size and programs computerized power contrast injection apparatus based on the same.
MAJOR DUTIES:
- SCRUB. Participates with physician in planning procedures then initiates preparation for procedure and independently either makes recommendations about procedures or as appropriate makes changes to procedures. Uses independent judgment in identifying and selecting patient/procedurespecific catheters supplies equipment and contrast medium and volume and then manipulates or changes supplies or instruments in order to meet the requirements of the procedure. Varies technique to accommodate patients condition and adapts methods to produce best results. Constantly researches new methods and techniques to perform safer and more efficient procedures then tests refines and implements new procedures. Prepares sterile field using aseptic technique and compounds medications/solutions such as heparinized saline nitroglycerin verapamil and others based on local SOP and physician orders. Performs both venipuncture and arterial puncture independently as a means to gain arterial and/or venous sheathed access. Maintains aseptic technique manipulates wires and catheters anticipates catheter tip location operates fluoroscopic singleplane and biplane xray equipment selects appropriate angulations positions xray table and image intensifier for optimal display of angiographic anatomy while simultaneously performing complex portions of the following procedures: selective vessel and heart chamber pressure recording; selective vessel and chamber blood sampling and/or shunt detection studies via oximetry; selective contrast angiography (aortic carotid coronary left ventricle peripheral vascular pulmonary and renal) via manual or power injection of radiopaque contrast media; coronary rotational atherectomy; coronary and peripheral intravascular ultrasound manual and mechanical thrombectomy angioplasty and stent implantation; foreign body retrieval from heart or great vessels; vena cava filter implantation; preparation and insertion of the Impella percutaneous left ventricular assist device; permanent pacemaker implantation and/or lead wire insertion; venous access device implantation; laser lead extraction; transvenous temporary pacing catheter insertion; thermodilution catheter insertion and cardiac output measurement; intraaortic balloon pump insertion; Impella Left Ventricular Assist Device (LVAD) insertion; peripherally inserted central catheter (PICC) central venous and arterial line insertion; endomyocardial biopsy (EMB); pericardiocentesis; cardiac structural procedures such as Transcatheter Aortic Valve Replacement (TAVR) Transcatheter Mitral Valve Repair Tricuspid Valve Repair Atrial Septal Defect (ASD) / Patent Foramen Ovale (PFO) closures left atrial appendage closure; drug response studies; pharmaceutical or exercise stress angiography; intracoronary anticoagulant thrombolytic and/or nitrate administration; wound closure and dressing application; blood drawing for point of care testing such as blood coagulation timing and blood oximetry; and hemostasis via manual compression external hemostatic device application and management to include radial compression devices FemoStop femoral compression device and hemostatic patches. Internal arterial and venous hemostatic device preparation and insertion to include AngioSeal Mynx Perclose Vascade and others.
- Initiates Advanced life Support (ALS) / Advanced Cardiac Life Support (ACLS) protocols as needed which includes performing CPR administering medications defibrillating patients as needed and all other ALS tasks.
- Compounds various pharmacological agents/medications using sterile technique and administers medications as directed by physician or independently under ACLS protocols; confirms dose and pressure of radiopaque contrast media; responsible for injection of radiopaque contrast media through manual or mechanical devices; assists with providing other medications as needed; and ensures provides close loop communication with nurse and monitoring technologist on medications given to patients.
- MONITOR. Reviews and/or records patient history and supporting clinical data prior to each of the procedures listed above. Simultaneously analyzes and controls highly specialized electrophysiologic angiographic and hemodynamic monitors with interactive complex programs. Monitors hemodynamic values waveforms pressure gradients and electrocardiogram tracings; immediately differentiates normal from abnormal and informs team of changes in patient status. Records anatomical physiological and/or pathological data for further diagnosis as well. Retrieves images for replay as well as roadmap. Performs detailed measurements to include quantitative and qualitative assessment of arterial stenosis ventricular volume ejection fraction wall motion and valve area. Calibrates and operates a vast array of specialized cardiac and vascular evaluative equipment to include IntraVascular Ultrasound (IVUS) Optical Coherence Tomography (OCT) Fractional Flow Reserve (FFR) Instantaneous Flow Reserve (IFR) and more. Keeps detailed records of procedures performed and related data. Updates the patient Electronic Medical Record (EMR) and other records obtained during the procedure. Identifies files and stores records and retrieves information from prior studies and the EMR as needed. Operates and maintains equipment for viewing medical records. Initiates ALS/ACLS protocols as needed which includes performing CPR administering medications defibrillating patients as needed and all other ALS tasks.
- CIRCULATOR. Attends to patients safety and comfort needs before during and after procedure; positions patient initially and as needed during the procedure to ensure optimal study results and patient comfort; connects and verifies functionality of physiologic monitoring
- equipment such as ECG blood pressure pulse oxygenation capnography and arterial and/or venous pressure transducer. Ensures proper function and data collection of all monitoring equipment and software during procedure. Manages cardiovascular imaging systems/cardiovascular picture archival communication systems to include but not limited to placing procedural orders uploading data ensuring complete transmission of data and images and creating post procedural DVDs. Anticipates the need for locates and opens sterile and non sterile supplies using aseptic technique. Operates cardiac defibrillator and pacemaker generator as well as radiographic contrast injector cardiac output intravascular ultrasound thrombectomy atherectomy intraaortic balloon pump and Impella consoles. Monitors and ensures compliance with aseptic technique and radiation safety principles. Compounds and administers medications as indicated by procedural protocol as directed by physician or independently under ACLS protocols. Repositions monitors and equipment as xray table and image intensifier are moved.
- Performs pointofcare testing such as activated clotting time and blood gas saturations; monitors patients response to intravenous sedation/analgesia medication administration. Maintains safe and clean lab environment during the procedure. Assists with application and management of external hemostatic devices to include radial compression devices FemoStop femoral compression device and hemostatic patches. Assists in orientation and effective functioning of float registered nurses (RN). Initiates ALS protocol as needed which includes performing CPR administering medications defibrillating patients as needed and all other ALS tasks.
- TECHNICAL SUPPORT. Performs technical support duties as assigned which may include but are not limited to contacts employees outside the immediate work unit patients their families physicians nurses other professionals and technical resource personnel to coordinate work efforts and resolve technical problems. Provides detailed patient education regarding pre intra and post procedural requirements expectations and potential complications. Prepares procedural site prior to procedure by shaving positioning and sterilizing site. Transports patients. Applies noninvasive cardiovascular techniques of electrocardiography (exercise stress test and 12lead ECG) and assists with synchronized cardioversion and transcutaneous pacing. Manages cardiovascular imaging systems/cardiovascular picture archival communication systems to include but not limited to placing procedural orders uploading data ensuring complete transmission of data and images and creating post procedural DVDs. Connects external imaging systems such as cardiac ultrasound cardiac device programmers IVUS OCT ICE and other imaging sources into cath lab suite display systems then manipulates display programming to appropriately display correct input based on procedural requirements. Performs inventory management; both within MHS GENESIS and through DOD systems such as DMLSS and Logicole. Maintains clean and safe work environment. Serves on hospital/service/unit committees and implements mechanisms to maintain or improve established standards. Provides technical and administrative support and advice to technicians nurses and physicians regarding new supplies equipment and procedures. Develops procedural instructions to insure proper performance of procedures. Participates in policy/procedure review and revision process. Collects and compiles data for formal/informal research efforts and submits to regulatory bodies as required locally. Maintains and calibrates complex diagnostic and interventional equipment. Conducts quality control checks; performs corrective actions as needed. Assists in controlling and maintaining supply inventories through detailed and regular supply inventories. Cleans instruments and maintains sterile instrument inventory. Practices and promotes compliance with Infection Control Radiation Protection and Fire and safety regulations. Prepares oncall schedule and participates in rotating 24hours/day 7days/week oncall duties.
- TRAINING. Instructor identifies and select different instructional resources and methods to meet student and preceptees varying needs; assists in instructing students and orientees with identifying and selecting patient/procedurespecific catheters supplies equipment and contrast
- medium and volume and rationale for various treatments and methods. Precepts on how to manipulate and changes supplies or instruments in order to meet the requirements of the procedure; monitor students in the use of learning materials and equipment use relevant technology to support instruction; observe and evaluate students performance and development; provide appropriate feedback on work; encourage and monitor the progress of individual students; communicate necessary information regularly to students colleagues and leadership regarding student progress and student needs. Trains new cardiovascular technicians nurses in all phases of operations related to the performance of cardiac catheterizations. Precepts instructs and evaluates students of the cardiovascular specialty course in all modalities of the cardiovascular field IAW established guidelines. 10%

- General familiarization with and knowledge of the Society for Cardiovascular Angiography and Interventions (SCAI) Society of Invasive Cardiovascular Professionals (SICP) and Cardiovascular Credentialing International (CCI) in order to keep abreast of best practices advances in cardiovascular treatment technologies and training opportunities
- Professional knowledge of cardiac anatomy and physiology to include complex congenital cardiac defects and principal physics associated with fluid flow dynamics.
- Professional knowledge of cardiac complex anatomy function hemodynamic and intravascular ultrasound principle practices and procedures sufficient to safely provide high quality care in high risk invasive procedures.
- Knowledge of research methods and procedures sufficient to provide clinical studies and

- Comprehensive and practical knowledge of anatomy and physiology of the heart lung and circulatory system including embryonic development and congenital malformations as seen in adult congenital patients.

- A thorough knowledge of diagnosis and treatment of congenital and acquired heart disease and peripheral vascular disease which requires new increasingly complex and rapidly changing technology of invasive cardiovascular examinations and procedures combined with either diagnostic or supportive medications and therapeutic interventions for which there are no standard instructions and procedures and which involve very fine distinctions or many delicate and exacting steps with complex instruments settings and measurements.

- Ability to troubleshoot equipment and make minor repairs or communicate the need for bioengineer repairs.
- The technologist reports directly to and receive primary instructions and supervision from the cardiac catheterization laboratory (CCL) Chief of Cardiovascular Interventional Services or Cardiology Service and is indirectly supervised by the clinical nurse officer in charge (CNOIC). During procedures incumbent has full responsibility for the technical aspects of routine and special invasive cardiovascular procedures. Employee resolves technical problems coordinates the work with others as necessary and keeps supervisor informed of potentially controversial matters or problems having farreaching implications. Uses lectures conferences and demonstrations as required to instruct and evaluate hands on skills of 16 cardiovascular students per annum in the cardiac catheterization laboratory. Work is evaluated by the supervisor for technical soundness appropriateness and conformance to policy and requirements through review of student progress as compared to norms occasional monitoring of instructional activities and feedback from clinical personnel. Physicians are not always readily available and incumbent must make difficult judgement calls based on findings of exam.
- Works on a collegial basis with RN and other team members to achieve
- Guidelines are primarily general in nature and require considerable interpretation. The guidelines include accrediting and certifying agency requirements manufacturers catalogs and specifications and professional journals and literature. Manuals manufacturers instructions and medical center directives or standard operating procedures (SOPs) are available but do not apply to every contingency. Selection of instruments for example may be based on knowledge of incumbents advanced knowledge of the equipment and on the cardiologists preference rather than on established guidelines. The incumbent is expected to use initiative and resourcefulness coupled with current knowledge and technical skill to deviate from established trends if required to provide a safe successful procedural outcome. Significant independent judgment is required to interpret and adapt guidelines to changing patient needs and the wide variety of situations encountered. Wellestablished procedures for doing the work are available but are not completely applicable to all work situations due to rapidly changing technology or the complexity of the patients disease and physical conditions. The technologist uses judgment to adapt and change procedures adopt or develop new procedures or techniques for individual problems.
- During the procedure constantly evaluates several indices of patient status i.e. invasive and non invasive blood pressure electrocardiogram stability spO2 to immediately detect need for emergency There are many situations calling for continual readiness to take a wide range on interventions. There are many treatment or care procedures occurring simultaneously and many possibilities for happenings that are life threating.
- The work requires regular and recurring physical exertion. It may involve walking frequent bending reaching and stretching to set up and take apart equipment; lifting carrying and positioning patients; Cardiopulmonary Resuscitation (CPR) while wearing protective lead aprons; carrying pushing or pulling moderately heavy objects; performing tasks that require repetitive wrist motion; and frequently but intermittently working in low light conditions. Duties require above average dexterity.
Minimum Qualifications:
* Education: Completion of an accredited Registered Cardiovascular Technician Certification (RCVT) program.
* Experience: The technician must have a minimum of four (4) years professional Cardiac Catheterization experience. The technician must have worked full time or part time (greater than 20hrs/week) as a professional Cardiac Catheterization technician for at least 24 months in the last 48 months.
* Life Support Certification: Current AHA or ARC BLS for Providers Certification.
* Security: Must possess ability to pass a Government background check/security clearance.
