Job Description
Job Title: Pharmacist Outpatient and Inpatient
Location: Albuquerque NM 87108
Start Date: April 2025
Duration: 6 12 Months
Shift Schedule: Rotational shifts (812 hours) including weekends and holidays
Qualification:
Education: The pharmacists must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited college or School of Pharmacy with a baccalaureate degree in Pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
License: Must have an active and unrestricted Pharmacist license in any U.S. state
Certifications: BLS Good to have
Experience: Minimum 2 years of Experience required in either Inpatient or Outpatient Pharmacist roles
Responsibilities:
- Dispense prescribed medications ensuring compliance with VA policies USP 797 and USP 800 standards.
- Counsel patients on drug interactions side effects and medication adherence.
- Review and verify all medication orders in the VISTA electronic medical record system.
- Maintain strict security inventory and documentation of controlled substances per VHA Handbook 1108.01.
- Perform sterile and nonsterile compounding in compliance with USP 797.
- Collaborate with physicians nurses and allied health professionals to ensure optimal medication therapy.
- Handle hazardous drugs safely and in compliance with OSHA regulations.
- Maintain patient privacy and adhere to HIPAA guidelines.
