We are seeking a Home Health Physical Therapist to join our client s team.
The Home Health Physical Therapist will be responsible for providing highquality physical therapy services to patients in the comfort of their homes. This role involves assessing patients needs creating personalized treatment plans and implementing interventions to enhance mobility alleviate pain and improve the overall quality of life. The ideal candidate will possess excellent communication skills strong clinical judgment and the ability to work independently within a multidisciplinary team.
Responsibilities:
- Conduct comprehensive assessments of patients physical abilities including range of motion strength balance and gait.
- Develop individualized treatment plans based on assessment findings patient goals and physician recommendations.
- Administer therapeutic exercises manual therapy techniques and modalities to promote functional independence and mobility.
- Educate patients and caregivers on proper techniques for exercises and daily activities.
- Monitor patient progress and adjust treatment plans as necessary to achieve therapeutic goals.
- Collaborate with physicians nurses and other healthcare professionals to ensure coordinated and continuous patient care.
- Maintain accurate and timely documentation of evaluations treatments and progress in electronic medical records.
- Ensure compliance with state regulations Medicare guidelines and agency policies.
- Participate in professional development activities to remain current with evidencebased practices and advancements in physical therapy.
Requirements
- Bachelor s Master s or Doctorate degree in Physical Therapy from an accredited program.

Current state licensure as a Physical Therapist.

Minimum of one year of experience as a Physical Therapist preferably in home health or rehabilitation.

CPR certification required.
