24 hr call 4 hrs gratis xx3300 and below
Callback: xx430 and below
Dealbreakers: if not fellowship trained in Ortho Trauma
Travel Preferences:
Preferred Lodging: Preferred hotels
Air Travel: Local no flights
Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Requirement description :
- Board Certified Required
- Active CA License Required
- BLS DEA during credentialing Current CA fluoroscopy permit REQUIRED by start date. Will consider those willing to obtain as long as they will have it by April 2025!
- Fellowship trained in Ortho Trauma is REQUIRED!
- Clean Malpractice Required
- List out availability at time of name clear Required
