You are the coordinator of care guiding patients through complex medical systems to ensure they receive the right care at the right time. You manage transitions collaborate with multidisciplinary teams and advocate for patients needs. Your role involves essing patient needs coordinating services and developing care plans that optimize health outcomes. Whether its arranging followup care or navigating insurance you make sure no detail is overlooked in your patients journeys. Your toolkit Care plans resource networks and patient advocacy. The facility is in WILSON NORTH CAROLINA (CA)
THE DEETS
This is a 13 week Travel/ Local contractand it has a Start Date of 21Oct24
THE SHIFTS
Working 12 hour shifts 3 shifts per week on a shift pattern
THE MUST AVS
You hold a valid Case Management certification (CCM) or equivalent and if required the appropriate valid state license.
THE PAY
HOURLY PAY: $48.00 SHIFT PAY: $672.00 WEEKLY PAY: $2016.00 TRAVEL TAX FREE: $1411.20
THE CALCULATIONS & SMALL PRINT: Illustratively the weekly pay is based on these patterns: 8hour shifts 5 days (40 hours) 9hour shifts 4 days (36 hours) 10hour shifts 4 days (40 hours) 12hour shifts 3 days (36 hours). For PER DIEM/ PRN shifts we have illustrated on a 3 shifts per week basis. When shift / weekly hours durations incur OT in any state then this is reflected in both the shift and weekly pay. **Travel tax regulations apply within the USA. The industry average pay for this role based on location and experience level has range of $1713.60 $2318.40
