Job Description
Summary: We are need of 24 hr ED provider coverage to start Nov 20th at 6am. This will be ongoing coverage and can have no gaps. Current schedule is two providers per 24 hrs working 12 hr shifts. Coverage is supplemented with employed APPs that work 10am to 10 pm .
First To Fill
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
(Daily) BR601BR Per 24 hours to include 4 hours gratis
Holiday Must be 1.5x the regular rates Per holiday hour no OTSchedule/Availability Requirements:

Reason for Coverage: Break in current contract
Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.
Air Travel: BR/round trip cap
Rental Cars: BR5/day all in cap (including taxes and fees) regardless of car class.
Mileage: IRS Standard Rate
Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

- Board Certified REQUIRED
- Active ND license or IMLC (LOQ in hand) REQUIRED
- ACLS BLS DEA ATLS REQUIRED
- Clean Malpractice/Background HIGHLY PREFERRED
- Must list availability(when can they start and how much coverage can they offer) REQUIRED
