Job Summary:
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide highquality nursing care to patients support the interdisciplinary care team and help ensure patients receive effective compassionate care tailored to their individual needs. This role is vital to fostering a supportive environment for patients families and healthcare staff.
Key Responsibilities:

Documentation and Communication:
- Accurately and thoroughly document patient care activities.
- Consult with and seek guidance from the Registered Nurse (RN) as needed.
- Participate in Utilization Management and Interdisciplinary Care Management Meetings as directed by the RN to support patient discharge planning.
- Effectively communicate discharge recommendations to the attending physician patient and family and assist in preparing them for the discharge process including providing patient/family education.

Patient Care Oversight:
- Oversee patient care provided by unlicensed staff assigning duties per State Nurse Practice Act company policy and staff training requirements.
- Communicate patient information with unlicensed staff during and between shifts and monitor and evaluate their responsibilities.
- Ensure all assigned tasks are performed in accordance with policies and procedures.

Clinical Duties:
- Use Point Click Care (PCC) for patient documentation and follow business processes.
- Perform drug count at shift changes and as needed.
- Assist physicians during rounds and examinations of patients.

Collaboration and Teamwork:
- Participate in shifttoshift communication to ensure smooth transition between incoming and outgoing nursing staff.
- Work closely with other departments to deliver timely and effective care that aligns with the patients preferences.
- Promote a respectful teamoriented environment that is responsive to the concerns of staff patients and families.

Professional Development:
- Enhance nursing practice by attending mandated inservice programs.
- Assist with orientation of newly hired nursing staff.
- Foster a culture of safety to ensure a healthy practice and living environment.

Compliance and Confidentiality:
- Adhere to policies ensuring confidentiality and protection of sensitive Protected Health Information (PHI) at all times.
Requirements:

Qualifications:
- Active Licensed Practical Nurse (LPN) license.
- BLS certification (or as required by state).
- Proficient in electronic medical record systems ideally with experience in Point Click Care (PCC).
- Excellent communication teamwork and timemanagement skills.
- Ability to work effectively in a fastpaced teamoriented environment.

- Minimum of 2 years of experience in a clinical or longterm care setting is preferred.
