Job Description
Job Title: Primary Care Physician
Location: Phoenix AZ
We are seeking a dedicated and compassionate Primary Care Physician to join our team. The ideal candidate will be responsible for providing primary medical care services to patients including diagnosing and treating a wide variety of conditions prescribing medications and coordinating care with specialists as needed. The Primary Care Physician will also be expected to promote health maintenance and disease prevention through patient education and wellness initiatives.
Responsibilities:
Conducting thorough patient assessments and medical histories
Performing physical examinations and interpreting diagnostic tests
Developing and implementing comprehensive treatment plans
Monitoring and managing chronic medical conditions
Providing patient education on disease prevention and healthy lifestyle choices
Collaborating with other healthcare professionals to ensure coordinated patient care
Maintaining accurate and uptodate medical records
Participating in quality improvement initiatives and continuing medical education

Medical degree from an accredited school of medicine
Board certification in Internal Medicine or Family Medicine
Valid state medical license and DEA registration
Strong clinical skills and knowledge of evidencebased practices
Excellent communication and interpersonal abilities
Commitment to providing highquality patientcentered care
Ability to work effectively in a fastpaced and teamoriented environment
