RN Case Manager with experience as follows:Must have strong acute care hospital discharge planning experience.Conducting patient initial assessmentsAcute Care hospital experienceEPIC EMR documentation experienceActive CA RN LicenseMonday through Friday 0800 to 1630.Eight hour shifts only.40 hours guaranteed.
Shift: Day shift () Monday thru Friday
