Job Description

Evolve Helthcare is seeking anRN Case Manager for our location in Carmichael CA.

Start Date 10/29
40/hours per week
Cerner Exp.
2 years CM RN Exp.
AHA BLS
CA RN License Active in good standings
Shifts depends on dept but hours are between 85pm

Job Details:

*****MUST HAVE PRIOR DIGNITY EXPERIENCE OR THEY WILL NOT CONSIDER*** Unit Case Management/Care Coordination # patients seen/consults per day 1520 Certs: BLS EMR: Interqual Weekend Requirement every other depending on staffing. Would be at least one full weekend per month Holidays not required but will work if they are willing.

All areas of the hospital will be covered no OP and no Home Care.

Dealing with all payers uninsured homeless. Completing Initial assessments IQ DC planning cerner

Additional Questions

Do you require case managers to be familiar with Interqual criteria yes

What other specific skills or knowledge are needed to be successful as a case manager at your facility collaboration teamwork effective communication ability to reprioritize throughout the day.

Are they required to do Utilization review as well yes

We will train candidates in IQ. Both DC Planning and UM are required.
