Job Description
Summary: Penrose is looking for an Ortho Hand Surgeon to provide locum coverage for upper extremity call for ACS Level 1 Trauma Center. Scheduling of provider is limited to 4 days per month ongoing with the ability to start November 2024.
BID DUE DATE: Sept 24th @ 5p EST!
Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Holiday Must be 1.5x the regular rate. Per holiday hour no OT
On Call (Daily) xx3601xx4400 Per 24 hours to include 4 hours gratis
On Call Daily Holiday Call Rate $5401.50 xx6600 Per 24 hours to include 4 hours gratis
Call Back xx451$500 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back xx676.50 xx750 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
- 24 hour call to start Nov 2024
- 4 days per month with weekend coverage as a preference.

EMR Required Training:
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Virtual training Only able to attend if Modules are completed before training. Virtual EMR training must be scheduled prior to the providers first day to avoid the provider missing their first day.
Travel Preferences (see attachment for specific travel guidelines):
Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.
Air Travel: xx750/round trip cap
Rental Cars: xx65/day all in cap (including taxes and fees) regardless of car class.
Mileage: IRS Standard Rate
Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

- Will they require ATLS for hand I know some of the other CS CO locations were requiring ATLS for level 3 gen ortho .do not need ATLS for Ortho

Requirement description : **MUST NOTATE ALL DURING NAME CLEAR TO AVOID BEING REDIRECTED**
- Board Certified Orthopedics Required or Truly Board Eligible (05 years of residency)
- Active CO license Required or IMLC with LOQ in hand at time of submission.
- Orthopedic Surgery Residency Required
- Hand Surgery Fellowship Highly Preferred
- Candidate must be available for 4 shifts per month Required to note.
- Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
- Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
- Must disclose what month provider is able to start Required
- Clean Malpractice / background Case by case must include malpractice explanation or self inquiry NPDB report at name clear.
