Overview
As a Physical Therapist in our clients Home Health team you will be responsible for providing Physical Therapy services to patients in their homes assessing patient functioning levels and developing treatment programs to improve independence and physical/cognitive functioning. You will also be involved in patient and caregiver education supervision and reporting on patient progress.

Responsibilities
- Perform Physical Therapy assessments and develop treatment programs
- Record and report patients reaction to treatment and condition changes
- Instruct patients and caregivers in therapy programs and device use
- Supervise physical therapy assistant and when appropriate the certified nursing assistant
- Prepare reports and participate in staff and Agency development activities
Qualifications
- Masters degree or higher
- Healthcare Setting (1 years)
- Valid Physical Therapist in the state of Illinois (IL)
- Valid Regular Driver License (DL)
Schedule and Shift Details
- MondayFriday; 8:00a/9:00a5:00p 1 weekend per month (Saturday & Sunday; day shift w/scheduled patients); 2 holidays (1 summer / 1 winter); no oncall
