Job Description
- Patient Care: Provide physical therapy services to patients in their home settings developing and implementing individualized care plans.
- Collaboration: Work with a multidisciplinary care team to ensure effective patient management and care coordination.
- Travel: Spend approximately 20% of your workday traveling to and from patient homes within the Dingmans Ferry area and surrounding locations.
- Documentation: Consistently meet deadlines for charting utilizing stateoftheart technology to streamline documentation and reduce time spent on administrative tasks.
- Mentorship and Development: Engage with clinical educators preceptors and supervisors for guidance and professional growth.
- Flexibility: Manage your own schedule with support from dedicated schedulers ensuring a balance between work and personal commitments.
- Required:
- Bachelors or Masters degree in Physical Therapy from an accredited institution.
- Licensed Physical Therapist in the state of Pennsylvania (PA).
- At least 1 year of healthcare experience.
- Proficient with Electronic Medical Record (EMR) systems including practicespecific EMR software.
- Valid driver s license and reliable transportation for local travel.
- Ability to manage time independently and meet critical charting deadlines.
- Preferred:
- Experience with any of the following EMR systems: Electronic Medical Record (EMR) Practice Partner EMR.
- Additional certifications or experience in home health or similar settings.
- Fulltime position: Monday to Friday 8:00 AM 5:00 PM.
- 1 weekend per month and 2 holidays per year required.
- No oncall duties.
- Local travel to patient residences in Dingmans Ferry and surrounding areas.
