Job Description
Job Summary:
Key Responsibilities:
- Provide physical therapy assessments and develop individualized treatment plans for patients in their homes.
- Implement therapeutic exercises and modalities to improve patients mobility strength and overall function.
- Educate patients and their families on exercises and techniques to enhance recovery and prevent further injury.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Document patient care activities in compliance with regulatory standards and organizational policies.
- Collaborate with interdisciplinary teams to coordinate care and support patient needs.
- Travel to patients homes within Butte Yuba and Sutter Counties; mileage reimbursement is provided in addition to the bill rate.
Qualifications:
- Current state licensure as a Physical Therapist in California.
- Bachelor s Master s or Doctorate degree in Physical Therapy from an accredited program.
- Minimum of 1 year of home health experience preferred.
- Strong communication organizational and interpersonal skills.
- Ability to work independently in a home setting while maintaining a strong focus on patient care.
