Pay Rate: Local$2000 to Travel$2200 Weekly
Unit: Home Health
Shift: 8 Hours; Days 8a 5p
Schedule: 40 hours / week guaranteed 5 shifts per week MonFri No weekends.
Position Requirements:
- Hospice Social Worker: 1 years Required
- Recent/Current Home Health social work experience: 1 year Required
- Home Care Home Base (HCHB) charting experience preferred
- Must have valid Drivers License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education License and Certification Requirements:
- Masters degree in Social Work (MSW) Required
- License (LMSW CSWA or LCSW) for State of Oregon Required
- BLS Certification from AHA or American Red Cross Required
Job Description:
- Provides rehabilitative and supportive social casework geared toward restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families in understanding accepting and following medical recommendations.
- Helps patients utilize the resources of their families and the community. This may be accomplished by either referring patients to resources or acting as a liaison/advocate on behalf of their patients in their dealings with other health and welfare agencies.
- Assists patients and their families with personal and environmental difficulties which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of the patients family to assisting patients with admission into facilitybased care.
- Consults with physician and other members of the health team for the purpose of assisting them to understand the significant social emotional and environmental factors related to the patients health problems.
- Prepares clinical/progress notes on the day of visit and incorporates them into the clinical record weekly; provides summaries and reevaluations if indicated.
- Attends case conference meetings.
- Participates in staff development and revision of physicians plan of treatment.
- Participates in staff development activities and inservice education.
- Assists in the development and revision of physicians plan of treatment.
- Participates in discharge planning and inservice programs.
