Apollo XCM is looking for the best Patient Care Coordinator to act as a leader in personal injury treatment plan coordination. This leadership role is crucial in ensuring exceptional patient care by overseeing treatment plans and facilitating seamless communication between patients caregivers and healthcare providers.
What youll be doing:
- Lead Patient Coordination: Serve as the main point of contact for patients providing empathetic and efficient support. Address patient inquiries manage appointments and coordinate followup care to ensure timely access to medical services.
- Collaborate with Teams: Work closely with interdisciplinary teams to streamline patient care coordination and improve overall outcomes.
- Educate and Empower: Guide patients and caregivers through treatment plans medications and selfcare strategies enhancing health literacy and empowering them to take charge of their health.
- Maintain Standards: Ensure accurate documentation of patient interactions care plans and progress notes in compliance with regulatory standards.
- Advocate for Patients: Identify and overcome barriers to care advocating for patient needs and connecting them with supportive services and community resources.
Requirements
Registered Nurse (RN) or MD license (US or Colombia).
Experience in healthcare coordination patient advocacy or customer service.
Bilingual proficiency (Spanish and English) with a required English level of C1 or above.
Strong multitasking and prioritization skills in a fastpaced environment.
Indepth knowledge of medical terminology and healthcare systems.
A compassionate patientcentered approach.
Attention to detail and proactive problemsolving.
Familiarity with startup culture.
