Director of Case Management
Position Summary:
Our client is seeking a Director of Case Management in Houston TX. This senior role requires leading and overseeing the local case management program including care facilitation utilization management and discharge planning. Responsibilities include developing processes managing budgets ensuring optimal resource use and promoting interdisciplinary collaboration.
Position Qualifications:
RN with CCM/ACM/FAACM certification
Current TX RN license
5 years in utilization management case management or discharge planning
3 years in hospitalbased nursing
5 years of supervisory experience
