Job Requirements

• We are seeking a compassionate and dedicated Medical Social Worker to join our team at Mamata Medical College And Hospital.

• As a Medical Social Worker you will be responsible for providing support and assistance to patients and their families during their medical journey.

• Your role will involve assessing patients social and emotional needs connecting them with necessary resources and providing counseling and support services.

Key Responsibilities:

• Conduct assessments of patients social and emotional needs

• Develop and implement care plans to address patients needs

• Connect patients with necessary resources such as financial assistance or support groups Provide counseling and support services to patients and their families

• Collaborate with healthcare team members to ensure holistic care for patients Maintain accurate and uptodate documentation of patient interactions and progress

Requirements:

• Bachelors or Masters degree in Social Work

• Minimum of 2 years of experience in a healthcare setting Strong communication and interpersonal skills

• Ability to work with diverse populations and handle sensitive situations with empathy and professionalism

• Knowledge of community resources and support services

• Proficiency in documentation and recordkeeping
