Position: Nurse Case Manager
Location: Remote with travel to sites in NW
Employment Type: Fulltime

My client is dedicated to delivering exceptional healthcare services to our community. We are looking for a dedicated and experienced Nurse Case Manager to join the team and contribute to improving patient care and outcomes.

Key Responsibilities:

• Develop and implement individualized care plans for patients.
• Coordinate and monitor healthcare services to ensure effective and efficient care delivery.
• Educate and support patients and their families about treatment options and care plans.
• Collaborate with healthcare providers social workers and other professionals to optimize patient outcomes.
• Maintain accurate and uptodate patient records and documentation.

Qualifications:

• Registered Nurse (RN) with a valid license.
• Previous experience in case management or a similar clinical role.
• Strong communication organizational and problemsolving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of healthcare regulations and best practices.
• Knowledge of CQC regulations and quality assurance processes
