Job Title: HCS Case Manager
Location: Angleton TX

About Us: Above and Beyond Caring is dedicated to providing highquality home and communitybased services (HCS) to individuals with developmental disabilities in Brazoria County. Our mission is to empower individuals to lead fulfilling lives by offering personalized support and care.
Job Summary: We are seeking a highly organized and compassionate HCS Case Manager to join our team. The ideal candidate will be responsible for coordinating and managing services for individuals enrolled in the HCS program. This role involves developing individualized service plans ensuring compliance with state regulations and advocating for the needs of our clients.
Key Responsibilities:
- Develop and implement individualized service plans (ISPs) for clients in collaboration with clients families and interdisciplinary teams.
- Conduct regular assessments and evaluations of clients needs and progress.
- Coordinate and monitor the delivery of services ensuring they align with clients ISPs.
- Advocate for clients to ensure they receive appropriate services and supports.
- Maintain accurate and uptodate client records including progress notes assessments and reports.
- Ensure compliance with all state and federal regulations as well as organizational policies and procedures.
- Facilitate communication between clients families service providers and other stakeholders.
- Participate in interdisciplinary team meetings and contribute to the development of comprehensive care plans.
- Provide training and support to direct care staff regarding clients needs and service plans.
- Assist clients in accessing community resources and supports.
- Respond to client crises and emergencies as needed providing support and intervention.
- Stay current with best practices and developments in the field of developmental disabilities and HCS services.
Qualifications:
- Bachelors degree in social work psychology sociology or a related field.
- Experience working with individuals with developmental disabilities preferably in a case management or similar role.
- Knowledge of HCS program regulations and requirements.
- Excellent organizational and time management skills.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid Texas drivers license and reliable transportation.
- Ability to pass a background check and drug screening.
