Job Description

The Transitional Living Coordinator will provide a coordination of specific services necessary to carry out needs related to the safe care and transitions of its consumers. Provide effective independent living services following outlined policies and procedures. Coordinating the responsibilities of the participating providers, facilities, and services. Focus on high-risk clients managing complex behavioral health, medical or multiple conditions assuring positive outcomes. Case management monitoring, evaluating and documenting all client activity. Serves as a liaison between client and resource agencies providing enhanced opportunities and placements. Assist with resources for obtaining benefits, transportation, personal care, medical services, employment or education, client transition as well as other individualized need areas.



Requirements

Valid Oregon Driver s License and insurable by the CTWS. Not engage in dysfunctional use of alcohol or any illicit drug use. Pass an alcohol and drug test. For people recovering from disease of substance addiction, continued sobriety for the previous two years. Pass a criminal background screening with Tribal, State, or Federal Law enforcement as required by PL 101-6630 (the Indian Child Protection and Family Violence Protection Act). Must be certified in CPR and First Aid (or be able to obtain within first 30 days of employment).
