Job Description

  • Manage home health, hospice, and bereavement social services programs.

  • Provide fiscal oversight and training.

  • Monitor volunteer activities and community relations.

  • Ensure compliance with regulations.

  • Supervise interdisciplinary team members for effective care plans.

  • Manage staff to meet standards, goals, and quality improvement.

  • Design, implement, and evaluate bereavement program services.

  • Lead outside presentations, workshops, and training sessions.

  • Participate in quality assurance and budget monitoring.

  • Travel about 10% of the time.



Requirements

  • Minimum three (3) years of experience in program management, supervision, or oversight responsibilities, as well as direct counseling work with individuals/groups, in a health care setting.

  • Minimum three (3) years of counseling experience.

  • Masters degree in counseling, social work or related field.

  • Current professional licensure (LCSW or MFCC), or certification.

  • Understanding of new and innovative organizational change processes.

  • Demonstrated expertise in working with grief /loss issues and crisis intervention.

  • Must be able to work in a Labor/Management Partnership environment.

  • Experience in budget management, program planning, program development, and strategic planning.

Education: Bachelors or graduate degree in Physical Therapy from an accredited university-based Physical Therapy program. Licenses/Certifications: Current license to practice Physical Therapy by Texas Board of Physical Therapy Examiners. Current Certification in Basic Life Support. A physical therapist practicing direct access: Education: Doctoral degree along with a residency or fellowship program, OR Licensed therapist with 30 hours of continuing competence activities in differential diagnosis. Licenses/Certifications: Texas Board of Physical Therapy Examiners license. Experience / Knowledge / Skills: At least two (2) years outpatient ortho experience required or completion of an accredited residency program. The position requires occasional/frequent lifting of up to 75/100 pounds and carrying of up to 30 pounds; prolonged standing; frequent walking, stooping and squatting. Fine hand dexterity, eye-hand coordination, the ability to receive oral and written communication, and to communicate verbally and in writing are also necessary. The visual ability to monitor instruments, computers and equipment, recognize alphanumeric characters, distinguish colors and observe physical characteristics are essential to this position.
