If so, Verovian Social Agency wants to hear from you for the role of a licensed social worker in discharge We are searching for a dedicated individual who is passionate about improving patients lives through efficient and effective discharge This temporary role is available in the Essex area and involves improving patient outcomes through effective discharge
About The Role
- Assessing the patients needs and potential barriers to successful discharge
- Collaborating with the interdisciplinary team to develop a discharge plan that meetsthe patients needs
- Connecting patients and their families with community resources and support services to ensure a smooth transition from the hospital or other health care setting
- Monitoring the patients progress after discharge and providing follow-up support as needed
- Advocating for the patients rights and needs in the discharge process
- Ensuring that all discharge planning is compliant with regulatory and ethical
Requirements
Candidate Requirements
Degree in Social Work or its equivalent
Enhanced DBS
Must be registered with SWE
Passport Page/ Eligibility to work in the UK
Proof of Address
Safeguarding Certificate
