Nurse Practitioner

Role Overview:
As a FacilityBased Advanced Practice Provider (APP) backed by Ingenuity Employment
Solutions you will contribute to the forefront of senior care within a postacute care setting.
Your responsibilities include administering direct patient care services aligning with our partner
organization's commitment to maintaining and enhancing each patient's clinical status and
quality of life.
Key Responsibilities:
Conduct Annual Wellness Visits for longterm Nursing Facility residents including
Annual History and Physicals as requested by Facilities.
Execute clinical interventional and episodic direct care visits with thorough reevaluation
of care visits.
Provide ongoing evaluation and treatment of chronic comorbid illnesses in line with
medical appropriateness.
Evaluate and treat residents for changes in condition.
Review medications orders and diagnostic tests promptly responding to clinical
abnormalities for all patients under your care.
Coordinate care with collaborating physicians for comprehensive medical management.
Assist with facilitywide illnesses leveraging appropriate collaborations.
Coordinate care with other disciplines such as Therapists Specialists and Social
Workers.
Maintain open communication with families and actively participate in family
conferences.
Fulfill other duties as assigned.
Qualifications:

Master's degree or higher.
Nursing Primary Care experience (2 years).
Valid licenses/certifications in either:
Nurse Practitioner (NP) in Iowa (IA)
Physician AssistantCertified (PAC) in Iowa (IA)
