Care Coordinators will:
- Review patient records to ensure that information is accurate
- Ensure that patient records/documents accurately reflect the current conditions/treatment requirements of each patient.
- Provide preventative care by identifying care gaps; Identifying Chronic Conditions
Requirements
CMA/LPN certificate/degree
Ability to work full-time (in office)
Empathetic and active listener; Detail oriented
Effective time management skills
Strong communication and interpersonal skills (written and verbal)
