Rate of Pay: 21.15 27.45 per Hour
Summary:
We are seeking a Supported Pathways Officer to assist residents in securing and sustaining their nextstage housing. This role involves providing clientcentered key working resettlement support and coordinating necessary services to ensure successful transitions. You will work closely with vulnerable residents to improve their physical and mental wellbeing and help them develop life skills that break the cycle of homelessness.
Responsibilities:
- Resident Support & Case Management: Provide assertive key working to a caseload of approximately 10 clients delivering holistic clientcentered assessments and support plans. Assist residents in securing and maintaining housing while ensuring they access appropriate services and activities to enhance wellbeing and life skills.
- Risk Management & Safeguarding: Conduct thorough risk assessments and manage cases in line with Adverse Childhood Experiences (ACEs) and TraumaInformed Care principles.
- Service Delivery & Compliance: Ensure compliance with the Care Act Housing Legislation and Council policies. Support residents in managing their accommodations including maintaining health and safety standards and assisting with practical tasks.
- Administrative Duties: Maintain accurate records of resident interactions risk assessments and case files. Manage rent and service charge accounts assist with benefit applications and monitor contractor performance. Prepare reports and contribute to regular performance reviews.
- Community Engagement & Advocacy: Advocate for residents by supporting them during appointments (e.g. health services DWP). Encourage residents to participate in social activities to reduce isolation and enhance community integration.
Qualifications:
- Knowledge of Adverse Childhood Experiences (ACEs) and TraumaInformed Care.
- Understanding of the Care Act Housing Legislation and available welfare support.
- Experience working with homeless people with complex support needs.
- Strong administrative skills and the ability to record and report information accurately.

- Ability to work independently and manage your workload effectively.
- Commitment to the Councils Equal Opportunities Policy and safeguarding standards.
- Willingness to undertake a Criminal Records Bureau (DBS) check upon appointment.
