Job Description

We are seeking a compassionate and skilled Wellness Nurse (LPN) for a parttime day position in our healthcare facility located in Southbury United States. As a vital member of our healthcare team you will provide holistic nursing care to our residents focusing on their overall wellbeing and quality of life.

  • Monitor the health safety and wellbeing of all residents
  • Provide direct nursing care with an emphasis on holistic wellness
  • Assist in training and monitoring medication administration
  • Supervise care staff in accordance with current state regulations and community policies
  • Maintain clinical quality assurance in line with federal state and local standards
  • Conduct thorough resident assessments
  • Collaborate with the Health and Wellness Director (RN) to ensure optimal care delivery
  • Promote the highest degree of service to our residents while demonstrating the mission of the company
  • Assist in the supervision of daytoday nursing activities performed by Certified Nursing Assistants
  • Ensure proper documentation of all nursing interventions and resident care

Qualifications :

  • Graduation from an accredited School of Nursing with a current LPN license
  • Minimum of 2 years experience in longterm care assisted living home health or hospital setting preferred
  • Experience working with residents with Alzheimers or other related dementias
  • Strong knowledge of federal and state regulations and nursing practices for health and wellness
  • Proficiency in computer skills including Microsoft Office (Windows Outlook Excel)
  • Excellent communication and interpersonal skills
  • Ability to handle multiple tasks and work efficiently in a fastpaced environment
  • Demonstrated ability to work independently and as part of a collaborative team
  • Current CPR certification
  • Physical requirements: ability to stand walk sit use fine and gross motor skills reach balance stoop kneel crouch and lift or carry weight up to 50 lbs
  • Flexibility to adapt to changing resident needs and work schedules
  • Empathetic and patientcentered approach to care
  • Detailoriented with a strong commitment to accuracy in medication administration and documentation
