Job Description
- Build and maintain professional relationships with staff physicians residents families community resources and thirdparty payers.
- Educate residents and families on Medicare Medicaid and financial assistance programs or refer them to the Finance Office.
- Complete admission forms assessments and required documentation.
- Develop and update individualized care plans tracking progress and adjusting as needed.
- Complete the Social Service section of the MDS within regulatory time frames.
- Gather Advance Directive information provide education and assist in completion if applicable.
- Continuously assess residents mental health needs provide education and facilitate referrals to mental health services.
Qualifications :
- Bachelors degree in Social Work required.
- MD Social Worker license required.
- 1 year of experience working in longterm care required.
