Job Description

Job Title: LTSS Service Care Manager

Location: Corpus Christi TX Remote with some field work

Duration: 6 months

Shift: Hours of operation is 85pm.

Responsibilities:

LVN needed to assist with follow up calls for members.
LVN will work assigned tasks
LVN may perform field visits in Corpus Christi area if needed
LVN will fill put required forms for each assessment

Required:

LVN license
Home health experience
