Job Description

Mobile Health Team Inc. a staffing agency has an opening for an Occupational Therapist(OT)Long Term Care. We are looking for someone with a passion for people.

Location: Cordova AK 99574

Assignment Start Date: 4/28/2025

13 weeks Varied 8hour shift 40 hours with 36 hours guaranteed weekly

This position description describes the general expectations of the staff Occupational Therapist; this is not intended to be an inclusive list of all the expectations of the person accepting this position.

The Occupational Therapist (OT) is responsible for the evaluation planning directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician. OT is responsible for directing the development implementation supervision and ongoing evaluation of the Long Term Care (LTC) activities program. This includes the completion and/or directing/delegating the completion of the activities component of the comprehensive assessment; and contributing to and/or directing/delegating the contribution to the comprehensive care plan goals and approaches that are individualized to match the skills abilities and interests/preferences of each resident.

OT will provide recommendations for the restorative nursing program. OT will contribute to and direct/delegate plans to nursing to provide individualized restorative care. OT will administer treatments and occupational agents as prescribed by a referring physician to restore function and prevent disability following injury disease or physical disability. OT will assist patients and residents to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.

ESSENTIAL RESPONSIBILITIES

  • Completes patient and resident evaluations and plans of program care.
  • Demonstrates competency in the performance of patient evaluations; cognitive perceptual sensory motor coordination prevocational function capacity; ADL; strength/range of motion; and home management assessments.
  • Evaluates work fitness and job analysis with followthrough to appropriate payer source.
  • Interview patients/families for program planning.
  • Selects testing & analysis and formulates long and shortterm goals.
  • Sets up and performs treatment plans.
  • Performs discharge planning for home modifications etc.
  • Treats patients of all ages: Pediatric/early childhood; Adolescent; Adult; and Geriatric.
  • Completes daily and weekly progress and summary reports.
  • Assists in the ordering and maintenance of supplies.
  • Maintains strict confidentiality regarding knowledge of all patient information (HIPAA) confidential staff and hospital business.
  • Establishes and maintains healthy interpersonal relationships with all staff members.
  • Completes chart audits to ensure appropriate timely documentation and completion of all charges.
  • Review LTC records to ensure that ALERT charting is completed.
  • Ensures that all daily weekly and monthly checklists and inventories are complete.
  • Works with the Billing Department for pre/prior authorization of admissions as needed.
  • Contributes to the setting of standards and monitoring of measurable standards of care and is accountable for maintaining those standards.
  • Follows all facility policies and procedures.
  • Assists with identifying and reporting incidents and accidents
  • Keeps uptodate with relevant clinical and professional developments in occupational therapy.
  • Is knowledgeable regarding State and Federal regulations for critical access hospitals and LTC facilities.
  • Performs other applicable tasks and duties as assigned.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

  • Minimum (2) two years of direct patient care in a variety of settings with a strong background in longterm care of geriatric patients.
  • Knowledge/experience with LTC RAI/MDS an asset

Education:

  • A valid Alaska state license to practice as an Occupational Therapist.
  • Graduate of an accredited program with a BS MA/MS in Occupational Therapy.

Licenses Certifications and/or Registration:

  • Must be Alaskalicensed in Occupational Therapy. Certification in Occupational Therapy by the National Board for Certification in Occupational Therapy (NBCOT).
  • Basic Life Support Certification and a valid Drivers License are required.
  • Must have an AK license to be considered

EMR System: CPSI/Evident experience preferred

Shift: MF 8 am5 pm

Knowledge/Abilities/Skills:

  • Professional friendly and warm demeanor. Must be able to change focus given added demands and or new responsibilities.
  • Must have excellent verbal and written communication skills and ability to work collaboratively as part of a team.
  • This position requires the ability to perform basic computing skills such as email word processing and spreadsheets.
