Hours: 37.5 hours per week
Salary: $29.66 $37.07 per hour
Shifts: Weekdays
Status: Temporary Full Time (1 year)

Position Summary
As an integral member of the Emergency Allied Health team the Resource Specialist helps manage and access resources. This new role aids in supporting completion of tasks that need follow up in the Emergency Department allowing patients to receive timely access to supports.
This is a new contract position in the Emergency Department at Toronto West Hospital. There is a possibility of extension or potential for becoming permanent.
Duties
- Providing assistance to patients/families/support persons and hospital staff on a consultation basis regarding related resources and other community resources (e.g. Trillium Drug Program Accommodation Transportation etc.)
- Assisting with application forms and letter writing (e.g. TDP Expedite MORRR ODSP EI CPPD Wheeltrans Immigration Visa Housing etc.)
- Staff education and updating resources used by staff and patients/families/support persons
- Acting as a liaison between the hospital and community agencies; assisting staff with program planning projects (e.g. developing education materials)
Qualifications :
- Social Service Worker Diploma
- Current Registration with the Ontario College of Social Workers and Social Service Workers
- 3 years relevant work experience
- Knowledge of Community and Government Resources (e.g. income replacement drug coverage transportation)
- Community health experience
- Experience with acute care preferred
- Teaching experience
- Familiarity with principles of Adult Education
- Presentation skills
- Strong Communication skills (written and verbal)
- Computer skills (e.g. Word Excel Outlook)
