Job Description

Union: NonUnion
Site:   Toronto General Hospital
Department:  Allied Health 
Reports to: Manager Allied Health
Hours: As needed
Salary: $42.21 to $52.76 per hour (To commensurate with experience and consistent with UHN compensation policy)
Status: Casual ( 2 positions )
Shifts: Weekdays
Closing Date: February 19 2025

 

Working under the direction of the Operational Manager and in collaboration with other members of the interdisciplinary team this casual social work position will support the inpatient and outpatient social work team (patient population to be determined based on department needs).

Duties

  • Assessing patients for the purpose of evaluating the need for service the nature of the problem and formulating recommendations or treatment plans
  • Implementing an intervention plan with patients family members significant others and the interprofessional team for the purpose of facilitating changes in behavior attitudes feelings and/or the environment
  • Reassessing and modifying the intervention plan as required
  • Liaising with appropriate community agencies
  • Completing applications for Alternate Level of Care (ALC) facilities establishing contact with members of the interprofessional team and community agencies to obtain provide or exchange information relating to patients psychosocial functioning and health care in the past present and future
  • Rendering services for the immediate benefit of the Hospital and the community
  • Performing activities required for the effective and efficient operation/maintenance of the Social Work professional group

Qualifications :

  • Completion of a Masters Degree in Social Work required
  • Registration with the Ontario College of Social Workers and Social Service Workers required
  • Membership in the Ontario Association of Social Workers preferred
  • Three (3) years experience working with an interprofessional team in a hospital setting preferred
  • Experience in acute care preferred
  • Experience in ambulatory care preferred
  • Excellent verbal and written communications skills
  • Excellent interpersonal time management and organizational skills
  • Excellent problem solving skills
  • Research experience/education an asset
  • Student education/supervision an asset
  • Knowledge of policies legislation programs and issues related to social welfare institutions and services
  • Knowledge of a second language an asset
