Job Description
Job Title: Senior LTSS Service Care Manager (RN)
Schedule: 8 am 5pm may require after hour visits as per member request
Location: El Paso TX
Duration: 6 months (possible Extension)
Responsibilities:
- Conduct inhome assessments including:
- SAI Core A to Core Q ISP EDP and other applicable forms (1500 SBP EVV).
- Assess members health status resource utilization treatment plans goals and provider options.
- Follow up on requests for services and speak with providers.
- Complete 5 successful visits per week.
- Submit assessments within 24 hours of visit.
- Submit all documentation within 5 business days.
- Travel to members homes for visits; travel time is accounted for in productivity.
- Complete documentation within required turnaround times and compliance guidelines.
Performance Expectations:
- Visit Completion: 5 successful visits per week.
- Documentation: Submit assessments within 24 hours all other documents within 5 business days.
- Work Hours: Typically MF 8 AM 5 PM; occasional evening or Saturday visits.
Required Skills/Experience:
- Registered Nurse (RN)
- Pediatric Experience
- Bilingual (English/Spanish)
Preferred Skills/Experience:
- Case Management
Education:
- Required: ADN or BSN
- Preferred: Bachelor of Science in Nursing (BSN)
Certifications:
- Registered Nurse (RN)
Additional Requirements:
- DFPS check: Not required
- Software skills: Proficiency in computer skills Excel
Travel Requirements:
- Travel to members homes including surrounding counties. Travel time is considered for weekly productivity.
