UHNs Transitional Care Program (TCP) includes UHNs Buttonwood Reactivation Care Centre (RCC) Hillcrest Reactivation Centre & Queens Estate Transitional Care units. The RCC aims to improve restoration to help patients maintain social cognitive and physical functioning by leveraging patientspecific goalbased service planning. Patients serviced by the TCP do not require acute care but need to engage in activities to improve their function and independence provided less active medical management and more therapy and activity. TCP also serves several health system purposes including:
- Reducing ALC pressures on acute care hospitals reducing the need for LongTerm Care Home placement and
- Decreasing the risk of readmission to an acute care hospital
Position Summary
ThePhysiotherapist(PT) is an integral member of the Physiotherapy service Allied Health clinical programs. Through the provision of patient and nonpatient care activities the PT enhances the quality of life of patients by planning and implementing individually designed programs of physical treatment to maintain improve or restore physical functioning and cardiorespiratory health alleviate pain and prevent physical dysfunction in patients.
Duties
- Gathers patient information including observing and interviewing the patient and essential care partners; reviewing and interpreting pertinent patient documentation and health records; attending and participating in rounds clinics and case management meetings; actively collaborates with interprofessional team member
- Conducts initial screening and both formal and informal assessment protocols to assess patients physical functional psychosocial perceptual and cognitive status
- Analyzes all collected data to develop a clinical impression formulate a treatment plan and consultation recommendations
- Communicates the results of the assessment/clinical impression to the patient essential care partners and health care team;if the results of the assessment indicate then communicates a diagnosis identifying a disease physical disorder or dysfunction as the cause of the patients symptoms as per the controlled act of communicating a diagnosis in the RHPA. Implements modifications and shifts in individualized treatment programs in order to reflect the patients changing needs as appropriate
- Coordinates and/or participates in the education of students/learners hospital staffand the community as well as maintaining excellence in the provision of physiotherapy services through ongoing professional development
- Leads and/or participates in research studiesrelated to physiotherapy
- Participates in the development implementation and monitoring of quality indicators as per the Quality Programs of Allied Health the Clinical Unit or Corporate Program of the University Health Network
- Critically appraises chooses and implements outcome measures to assess the effectiveness of physiotherapy interventions
Qualifications :
Qualifications
- Masters Degree in Science (Physiotherapy) or recognized equivalent based on Registration with the College of Physiotherapists of Ontario
- Registration with the College of Physiotherapists of Ontario
- Minimum One (1) year related experience
- Excellent interpersonal skills
- Excellent verbal and written communication skills
- Complies with the legislation outlined in the Regulated Health Professions Act (1991)
- Complies with the Code of Ethics and Rules of Conduct of the Canadian Physiotherapy Association
- Must be able to perform the physical demands of the job
- Demonstrates competence with departmental and hospital safety emergency and infection prevention and control procedures
- Recognizes and acts within the scope and limitations of the physiotherapist
- Ability to exercise sound clinical judgment and to function in an independent selfdirected manner
- Membership in the Canadian Physiotherapy Association preferred
