Job Overview
We are a Healthcare Consulting & Staffing firm for Hospice Homehealth and homecare. Currenlty our Georgia hospice client is seeking a dedicated and compassionate Registered Nurse Case Manager to join their healthcare team. In this role you will be responsible for coordinating patient care ensuring that individuals receive the appropriate medical services and support throughout their treatment journey. You will work closely with patients families and healthcare providers to develop and implement effective care plans tailored to each patients unique needs.
- Travel 100% required within service area for organization. If you do not like to drive this job is NOT for you.
Responsibilities
- Assess patients medical histories and current health status to develop comprehensive care plans.
- Coordinate with multidisciplinary teams to ensure continuity of care across various settings.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Provide education and support to patients and their families regarding treatment options and resources.
- Facilitate discharge planning to ensure a smooth transition from inpatient to outpatient care.
- Advocate for patients needs within the healthcare system ensuring they receive appropriate services.
- Document all patient interactions assessments and care plans accurately in electronic health records.
- Maintain uptodate knowledge of best practices in case management and relevant healthcare regulations.
- Must hold current uncumbered RN license to practice in Georgia.
- Must have current BLS
- Reliable transportation and auto insurance reqired

Skills
- Strong clinical background in areas such as Primary Care MedicalSurgical Infectious Disease Care Inpatient Care Behavioral Health PICU Managed Care and Discharge Planning.
- Excellent communication skills for effective interaction with patients families and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with attention to detail for managing multiple cases simultaneously.
- Proficient in using electronic health record systems for documentation and patient management.
- Compassionate approach to patient care with a focus on improving health outcomes.
- Hospice Experience Required
- HCHB EHR experience preferred

Pay: $88000.00 $96000.00 per year
