Job Description



Summary: Holy Cross Davis Hospital (HCD) is looking for a locum OB/GYN provider to help backfill for all gaps in coverage due to the loss of a few internal providers who have moved. Please make sure your the candidate provides the number of shifts per month the candidate can offer to the team on an ongoing basis. Site is looking for the provider to start as soon as credentialed.

BID DUE DATE: Jan 10th at 5p EST!

Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly BR or below Per hour not OT

Holiday Must be 1.5x the regular rates Per holiday hour no OT

On Call (Daily) BR150 or below Per 24 hours to include 4 hours gratis

On Call Holiday Call Rate BR100 Per 24 hours to include 4 hours gratis

Call Back BR or below Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Holiday Call Back BR Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Average worked Callback Hours: Low callback expected


Schedule/Availability Requirements:

  • Clinic hours are MonFriday 9a5p. Clinic coverage needed is Mon Tues & Thurs 9a5p.
  • Wednesdays include 24 hour call surgery assist coverage (7a7a).
  • 24 hour call includes Mon Wed & 2 weekends Friday (7a) Mon (7a) per month.


Details:

  • Looking for a provider to start as soon as credentialed
  • Clinic hours are MonFriday 9a5p. Clinic coverage needed is Mon Tues & Thurs 9a5p.
  • Wednesdays include 24 hour call surgery assist coverage (7a7a).
  • 24 hour call includes Mon Wed & 2 weekends Friday (7a) Mon (7a) per month.
  • Setting: Outpatient inpatient Emergency Room Clinic Call
  • Duties: OBGYN
  • Days per month: 7 shifts per month needed.
  • Required Procedures: Will include Gyn and OB patients with surgeries at HCD as well as assist in surgery.
  • EMR: EPIC
  • Support Staff: MA
  • Reason for Coverage: We had 2 OBGYN MDs move out of the state and we are currently looking for replacement. We need help with call until we are able to employ 2 new MDs.
  • Dealbreakers: Must be available for all functions of the job description.

EMR Required Training:

  • Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
  • EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed before the provider s first day and is only available MonFri for scheduling.
  • All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission before an offer can be made:
    • What are the # of years of epic experience they have
    • When was the last time (MM/YYYY) that they used Epic
    • What is the name of the last facility the provider used EPIC
    • Can you provide me with a photo large enough to add to a badge and not become pixelated
    • Can you disclose the providers time zone for scheduling the virtual epic training
    • We need the providers best phone # to use for call shifts while working. Can you disclose
    • We need the providers best email to use for programs while working onsite and the preapplication. Can you please send us their personal email address

Travel Preferences:

  • Preferred Lodging: Must follow CSH travel policy
  • Air Travel: Air travel okay. Local providers will be placed in preference of nonlocal providers.
  • Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Credentialing Information:

  • Timeframe: 6090 days after receiving completed application (Completed Application needs to be turned around in 5 days or less)
  • Requirements Must have active UT license to release preapplication. Must have active UT license CSR and DEA to release full application.

Additional Details / Questions Answered by Facility:

  • Admissions per day Patients/day Types of cases Disclosures that may need to be made etc.
  • Total # of beds 220 total beds at Holy Corss Layton.
  • For call shifts: We have an after hours phone system that will call the provider if they need to speak with a patient. The hospital will also reach out to the providers cell phone if anything is needed. The provider will need to stay within 20 minutes of the hospital while taking a call shift.
  • How many patients will a provider typically see in a clinic day Every provider is different however they typically see between 1535 patients per clinic day. Our locums though normally hae gotten up to 15 patients at the most in a clinic day.
  • Do you have the average call back hours on a call shift for the call back when on call it varies frequently from 02 times.
  • Does this facility have DaVinci robot Yes they do have the robot.

Requirement description : **MUST NOTATE ALL DURING NAME CLEAR TO AVOID BEING REDIRECTED**
  • Board Certified OB/GYN (ABOG) Required
  • Active UT license Required or IMLC (LOQ in hand at time of submission)
  • Active BLS CSR Required for credentialing must note willing to obtain if they do not have one.
  • Active DEA Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
  • Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date Required
  • Candidates must be available for a minimum of 7 days per month and must be able to start March 2025 Required
  • Candidate must be able to perform all functions of the job description to be considered and are required to note at presentation Required
  • Clean Malpractice Required willing to accept up to BR00k total in MP claims Must include self inquiry NPDB report at offer to be considered.
  • Clean Background Required

Weekend Requirements : Yes Friday (7a) Mon (7a) 2 weekends per month
On Call Requirements : Yes Mondays Wednesdays and 2 weekends per month.
