Job Description

RN Case ManagerEmergency Department FullTime Days

Hospital coverage: Northwestern Medicine Kishwaukee Hospital and Northwestern Medicine Valley West Hospital.

The Case Manager RN ED reflects the mission vision and values of Northwestern Memorial adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Expedite safe discharge plans quickly.
  • Evaluate patients for discharge needs to SAR SNF Home with Home Health or Acute Rehab from the ED.
  • Assists with hospice coordination from the ED.
  • Resource referrals to appropriate agencies teams and services.
  • Assists with followup appointments.
  • Manage high utilizer ED patients and patients with frequent admissions.
  • Coordinates appropriate DME for patients.
  • Works collaboratively with the health care team to prevent readmissions.
  • Assess patients to be admitted for discharge needs.
  • Concurrent review of patients clinical information for efficiency and correct level of care and admission status.
  • Interface with payers concurrently to obtain authorization for alternate levels of care.
  • Collaborates with the health care team and payers to assist with coordinating patients discharge planning needs.
  • Employ effective use of knowledge critical thinking and skills.
  • Work in an intensive fastpaced environment with minimal supervision.
  • Ability to stay organized and interact well with others in any situation.
  • Provide updates to Director of Care Coordination for review.

Qualifications :

Required:

  • Bachelors degree in Nursing
  • State licensure as a Registered Nurse (RN)

Preferred:

  • Masters of Nursing degree
  • Minimum 2 years of prior experience in Case Management
  • Utilization Management or Case Management Certification
