Dedicated Caregiver

Position Summary:

The Dedicated Caregiver provides a variety of personal care services to patients in their homes under the supervision of a Registered Nurse. These services are designed to assist patients with activities of daily living promote independence and improve their overall quality of life.

Essential Duties and Responsibilities:

• Provide personal care assistance to patients including bathing grooming dressing and toileting.
• Assist with mobility including transfers ambulation and use of assistive devices.
• Monitor vital signs (blood pressure temperature pulse respirations) as directed.
• Assist with light housekeeping tasks such as meal preparation laundry and changing bed linens.
• Provide companionship and emotional support to patients.
• Observe patient condition and report any changes to the Registered Nurse.
• Maintain accurate and timely documentation of services provided.
• Adhere to all company policies and procedures including infection control and safety guidelines.
• Participate in continuing education and inservice training programs.

Qualifications:

• High School Diploma or GED required.
• Certified Nursing Assistant (CNA) certification preferred but not required.
• Prior experience in home health care preferred.
• Excellent communication and interpersonal skills.
• Compassionate and patient demeanor.
• Ability to work independently and as part of a 1 team.
