Roles and responsibilities
Responsible for assessment of psychosocial functioning and capacities, counseling for care planning and decision making, social work treatment, including support through the diagnosis and treatment phases of care and ensure that all duties are carried out to the highest possible standard. The Social worker participate in the provision of teaching and supervision of peers on wide range of subjects within the clinical field.
Key Responsibilities / Accountabilities
- Conduct comprehensive psycho-social and family assessments by methods such as interviewing the patients/ family, obtaining relevant information, gathering social data and formulation of assessment and plan of intervention, in accordance with professional practice standards and clinical policies.
- Support the patients and their family by understanding the nature and treatment of the presenting issues by providing education, short-term counselling and crisis intervention to families and other relevant caregivers.
- Provide comprehensive clinical counselling with individuals, families and groups by focusing on the patient’s social, emotional and cultural needs, including adjustment to diagnosis and treatment decisions, loss of functioning and dealing with emotional and family crises in the context of evidence based care.
- Support the patients and their families by organizing and facilitating psycho-educational and counselling groups.
- Provide total individualized patient care as assigned according to the core standards of patient care and established policies and evidence based practices.
- Assume accountability for assessing, planning, implementing and evaluating psychosocial needs as well as discharge planning for each patient, assessing for current and potential problems
- Contribute to decreasing the length of stay/waiting time as per unit specific goal to achieve goals.
- Document all relevant information correctly, appropriately and in a timely manner and identify and alert members of the medical team to changes in medical status of the patient.
- Educate the patient/family and other members of the healthcare team regarding the social work services plan.
- Report equipment failures, safety and security issues, and/or reduction of supply inventory to Social Work Services Supervisor or designee.
- Promote patient satisfaction as a team member by demonstration of extraordinary service
- Promote a safe environment and safe patient care by embracing a safety culture and identify areas of risk to Social Work Services Supervisor. Comply with policies for personal and patient safety and for the prevention of healthcare associated infections.
- Contribute to the achievement of cost efficiency savings for the service. Maintain an awareness of resource utilization, exercising care in the ordering and use of staff, equipment and materials.
- Contribute to excellence in outcomes, a positive patient experience and a healthy professional environment. Contribute to the achievement of clinical quality outcome indicators and patient satisfaction.
- Liaise and work within an inter-professional practice model to ensure high quality, holistic, and safe family centered care and promote and share knowledge via mentorship/preceptorship programmes in line with relevant research and evidence based practice.
- Treat colleagues, patients, and visitors with dignity and respect at all times.
- Actively welcome/ orient new staff/students to the unit. Contribute to the retention of all staff to meet the service specific turnover rate goal.
- Uphold the organizational values at all times and follow all Hospital related policies and procedures.
- Support patient relations/ services activities to ensure a high level of satisfaction in the delivery of quality care and services to patients and their families.
- Participate in person-centered care initiatives undertaken by HMG.
- Enrich patient experience with compassion, respect and dignity.
- Perform other applicable task and duties assigned within the realm of his/ her knowledge, skills and abilities.
Desired candidate profile
Education/ Professional Qualification
Educational : Bachelor’s Degree
Professional : Any discipline. Preferably degree in Social Work
Certification : Relevant certifications
Experience
Preferably One (1) year’s relevant experience
Client-Centered Care:
Strong ability to build rapport and trust with clients from diverse backgrounds and varied life experiences.
Skilled in developing individualized care plans, including identifying client needs, setting goals, and coordinating services.
Ability to provide emotional support, counseling, and therapy to individuals and families facing challenges such as poverty, abuse, mental illness, or addiction.
Case Management:
Proficient in managing multiple cases, ensuring follow-up, and providing ongoing support to clients.
Experience in advocating for clients to access necessary services, benefits, and support systems, such as healthcare, housing, and employment assistance.
Skilled in tracking progress, documenting case details, and preparing reports as required by regulatory bodies or agencies.
Crisis Intervention and Problem-Solving:
Ability to respond to crises calmly and effectively, offering immediate support, stabilization, and guidance.
Experience in conflict resolution, de-escalation techniques, and addressing urgent emotional, mental, or situational needs.
Knowledge of available resources for crisis intervention and the ability to coordinate appropriate responses in emergency situations.
