Job Description
As a Virtual Intake Triage Registered Nurse you will play a key role in providing virtual health assessments triaging patients and coordinating primary care services in a fully virtual healthcare setting. This role is vital in ensuring that patients receive timely and appropriate care based on the severity of their conditions. Youll work closely with other healthcare professionals to develop patientcentered care plans and provide followup support ensuring a seamless care experience for every patient.
Key Responsibilities
- Conduct virtual assessments to evaluate general health concerns including acute and chronic conditions.
- Prioritize cases based on symptom severity and urgency ensuring timely coordination of care.
- Collaborate with primary care physicians and other healthcare providers to develop care plans.
- Refer patients to specialized care diagnostics or followup services as needed.
- Provide patient education on selfcare medication management and preventive health measures.
- Use telehealth platforms to deliver efficient and compassionate virtual care maintaining accurate documentation of patient interactions.
Qualifications :
You are a dedicated and experienced Registered Nurse with a passion for delivering primary healthcare in innovative ways. You excel in virtual care environments have excellent communication skills and can work collaboratively with a multidisciplinary team.
- Qualifications: Bachelors degree in nursing with current AHPRA registration.
- Experience: Minimum of 5 years in clinical program management ideally with a focus on primary healthcare triage or case management.
- Skills: Proficiency in virtual care delivery including telehealth platforms and electronic health record systems. Strong knowledge of primary care principles and chronic disease management.
