- Participates as an active member of the interdisciplinary team determines nursing goals of the care plan and monitors the participants progress toward goals
- Provides skilled nursing care and prescribed treatments to participants in their home
- Directs and supervises the provision of quality paraprofessional clinical care to participants in their home
- Conducts customer service home visit assessments of each new participant within the first week of program enrollment and performs reassessments and evaluations of each participant
- Assist with referrals to other services when participant conditions or situations require the service of other professional disciplines or the products or services of outside companies or agencies
Qualifications :
- RN or BSN degree with current RN licensure in the Commonwealth of Pennsylvania
- Current Pennsylvania Drivers License
- Thorough knowledge of current community health nursing practice and skill in the application of current nursing procedures
- Ability to effectively plan and coordinate nursing care for individuals families and groups
- Ability to work independently and as part of a team under pressure while maintaining cooperative working relationships
- 1 year experience working with the frail and elderly
