Job Description

The Center for Health Equity and Community Wellness (CHECW) aims to eliminate racial inequities resulting in premature mortality with a focus on chronic disease by addressing the social and environmental factors that impact health. CHECW works to increase placedbased investments in priority neighborhoods with community programming and services based on epidemiology influence and leverage the health system to promote wholeperson care; intensify the agencys approach to tackling big salt sugar and tobacco; and finding innovative ways to improve the built environment and address other social dominants of health. CHECW is of the Bureau of Bronx Neighborhood Health the Bureau of Brooklyn Neighborhood Health the Bureau of Harlem Neighborhood Health the Bureau of Chronic Disease Prevention the Bureau of Health Equity Capacity Building the Bureau of Equitable Health Systems and the Bureau of Finance Administration and Services.

The Bureau of Brooklyn Neighborhood Health: Formerly known as the Brooklyn Neighborhood Health Action Centers the Bureau of Brooklyn Neighborhood Health will continue to provide oversight and leadership in programming and planning for the Brownsville Neighborhood Health Action Center with satellite programming and planning also occurring at the Bedford and Bushwick District Health Offices. This bureau will support programming planning and research opportunities to address racial inequities resulting in premature mortality in Central Brooklyn. One important area of focus is perinatal health especially eliminating the inequities in maternal and child health outcomes that currently exist in New York City. CHECWs birth equity work is centered in three Bureaus of Neighborhood Health (Bronx Harlem and Brooklyn) which partner with community stakeholders to address health inequities and the drivers of premature mortality. The Bureau of Brooklyn Neighborhood Health seeks to hire a Doula Community Liaison to work in the Bronx location (1826 Arthur Avenue)

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
Building and maintain relationships with clinical and social service providers in the assigned catchment area and provide direct outreach on site.

Conduct outreach activities for the FWS CDI and Breastfeeding/Lactation Equity Initiative through tabling at events street outreach and presentations to community members and providers;
ensure that relevant flyers brochures incentives and other outreach materials are always on hand.

Facilitate referrals and coordination of care for perinatal health programs; develop resources for health and social service needs that are identified by the CDI doulas BNH health promoters and other community engagement and partnership staff.

Support direct recruitment of clients and recruitment of community members to be trained as doulas Community Breastfeeding Educators and Certified Lactation Consultants.

Represent the Health Department at community board meetings town halls and public meetings to promote programming and community engagement.

Facilitate monthly outreach meetings for BNH perinatal health programs and initiatives.

Collaborate on the Coordinated Intake and Referral system as part of the Citywide Doula Initiatives.

Other duties as assigned.

PREFERRED SKILLS:
Strong understanding of and commitment to birth equity .
Experience in outreach and recruitment for a communitybased doula health or socialservice organization.
Comfortable with direct outreach at clinics social service agencies and in the community.
Strong coordination and organizational skills.
Strong written and oral communication skills.

Qualification Requirements
1. High school graduation or equivalent and three years of experience in community work or community centered activities in an area related to duties described above; or
2. Education and/or experience which is equivalent to 1 above.
