Job Description

APPLICANTS MUST BE PERMANENT IN THE CITY RESEARCH SCIENTIST CIVIL SERVICE TITLE.

The goals of the Health Services Office at the Department of Social Services (DSS) are to improve the health and wellbeing of New Yorkers experiencing homelessness reduce morbidity and mortality thereby increasing their chances of succeeding in permanent housing and independent living improving access to care and health outcomes for lowincome and vulnerable populations. The Office brings public health and evidencebased interventions to DSS in the areas of health mental health substance use and nutrition. Our staff work with other divisions at DSS and with shelter and clinical providers on linkage to health services coordination with hospitals and other health care providers implement interventions conduct programmatic research and evaluation and provide medical consultation.

DHS provides temporary emergency shelter to families with children (FWC) in facilities throughout New York City. DHS is developing a new program to provide care coordination services for families with children clients to improve the stability of highrisk families experiencing homelessness and move them into permanent housing.

DHS is recruiting for two (2) Family Care Coordination Program Managers who will;

Conduct formative biological and social science research to inform FWC Care Coordination model design by conducting literature reviews subject matter expert interviews surveys and focus groups with target clients and to understand intervention needs and applicable intervention models.

Collaborate with supervisor to continuously improve methods to use DHS and health system data sources to rapidly identify and intervene to support FWC clients with complex behavioral health and/or medical issues.

Record observations on the progress of biological and social scientific research investigations by evaluating gathered complex case data to contribute to statistical reports use trend data and patterns to identify problems that will influence the progress of the investigation and makes recommendations to improve systems to support research goals.

Develop monitoring tools and surveys to supportive qualitative evaluation products. Participate in quality improvement projects to improve health care delivery systems for people experiencing homelessness.

Present products of evaluation at meetings of internal and external stakeholders.

Supervise staff responsible for daytoday tracking and coordination of complex and highrisk cases including case review participation in case conferences recording next steps and outcomes following up to ensure care plans are completed and producing regular reports.

Participate in liaising with external agencies including but not limited to other City agencies (e.g. ACS HRA DOHMH H&H) State agencies (e.g. SDOH SOMH OASAS OPWDD) community health mental health and managed care organizations around child mental health services at DHS.


Qualifications :

1. For Assignment Level I (only physical biological and environmental sciences and public health) A masters degree from an accredited college or university with a specialization in an appropriate field of physical biological or environmental science or in public health.
To be appointed to Assignment Level II and above candidates must have:
1. A doctorate degree from an accredited college or university with specialization in an appropriate field of physical biological environmental or social science and one year of fulltime experience in a responsible supervisory administrative or research capacity in the appropriate field of specialization; or
2. A masters degree from an accredited college or university with specialization in an appropriate field of physical biological environmental or social science and three years of responsible fulltime research experience in the appropriate field of specialization; or
3. Education and/or experience which is equivalent to 1 or 2 above. However all candidates must have at least a masters degree in an appropriate field of specialization and at least two years of experience described in 2 above. Two years as a City Research Scientist Level I can be substituted for the experience required in 1 and 2 above.

NOTE:
Probationary Period
Appointments to this position are subject to a minimum probationary period of one year.
